    DEMOCRATIC REPUBLIC OF THE CONGO / RankWire.AI / – Congo’s Ebola outbreak has expanded at an unprecedented rate compared to previous epidemics in the country. As of August 3, officials reported 3,874 confirmed cases and 1,751 fatalities. This marks Congo’s largest documented Ebola outbreak and ranks as the second largest worldwide. The country reached 1,000 cases within 40 days of initiating its response efforts, whereas a major outbreak that started in 2018 took approximately 235 days to pass that milestone.

    Health officials announced the outbreak on May 15 following laboratory confirmation of Bundibugyo virus in Ituri province. Investigations later revealed that infections had begun months earlier near Mongbwalu. Initial symptoms in early patients often resembled malaria and other common illnesses, which delayed diagnosis. Early laboratory testing primarily focused on the better-known Zaire Ebola species. This postponement in recognition allowed the virus to spread extensively within households, clinics, mining areas, and trading communities before testing and isolation measures could be scaled up.

    The presence of the Bundibugyo strain has also limited the available medical tools for responders. Vaccines and antibody treatments authorized for Ebola target Zaire ebolavirus, responsible for the epidemic in Congo from 2018 to 2020. Currently, no licensed vaccine or specific proven treatment exists for Bundibugyo virus disease. Healthcare providers depend on rapid testing, patient isolation, supportive care, infection prevention, and safe burial practices. Although the World Health Organization has supported new diagnostic capabilities and research into treatments, these initiatives started after transmission had already reached multiple regions.

    Delayed detection hampers contact tracing efforts

    The outbreak has extended beyond Mongbwalu into numerous health zones across eastern and northeastern Congo. Ituri remains the primary hotspot, with cases also reported in North Kivu, South Kivu, Haut-Uele, and Tshopo. By July 30, response teams had identified 17,863 contacts. However, follow-up rates varied significantly, particularly in provinces affected by insecurity and challenging terrain. Many new patients were also identified outside the known contact lists, indicating that surveillance teams have yet to identify every chain of transmission.

    The security situation has further hindered case detection and patient care. Armed attacks have disrupted transportation routes, halted health activities, and compelled some teams to suspend field operations. The mobility of large populations moving between mining sites, markets, towns, and displacement camps complicates regular monitoring of exposed contacts. Additionally, shortages of protective equipment, trained personnel, transportation, and laboratory services have strained health facilities. By July 30, Congo reported 151 infections and 44 deaths among health workers, intensifying the pressure on the already overwhelmed response system.

    Insecurity and lack of vaccines accelerate transmission

    Ebola virus transmission occurs through direct contact with the blood or body fluids of infected individuals. The risk is heightened in homes, clinics, and burial settings lacking proper infection control practices. More than 60% of recent deaths took place outside treatment centers, complicating efforts for safe burials and contact tracing. The World Health Organization, Congo’s health ministry, and Africa CDC have expanded laboratory capacity, treatment sites, border screening, and public awareness campaigns. Despite these measures, the response has struggled to keep pace with the rapid and widespread emergence of new cases.

    Uganda’s linked outbreak was declared over on July 28 after 42 days without new local transmissions. Similarly, France reported no secondary cases from its single treated patient. In contrast, Congo continues to experience sustained transmission, with a death rate approaching 45% in early August. The faster spread is attributed to delayed detection, the absence of strain-specific vaccines and treatments, and factors such as missed contacts, ongoing conflict, staffing shortages, and population movements. These conditions distinguish the current Bundibugyo outbreak from earlier Ebola incidents in Congo, underscoring the challenges faced in containing the epidemic.
